Atrial Fibrillation
Brief History:
Mr. X is a 72 year-old man who has a known history of paroxysmal atrial fibrillation. After a transesophageal echocardiogram was performed and revealed no contraindications to cardioversion, the decision was made to proceed ahead with an attempt at cardioversion.
Procedure:
Informed consent was obtained. NPO status for the past 8 hours was confirmed with the patient. A time-out was performed. Sedation was administered via anesthesia with Propofol and adequate sedation was achieved. The patient was placed in a supine position with Zoll pads appropriately placed anteriorly and posteriorly. Synchronized cardioversion was attempted with 75 joules and was successful in terminating the atrial fibrillation and resuming sinus rhythm which was confirmed by the rhythm monitor.
The patient was aroused from sedation and the Zoll pads removed. The patient's family member was brought into the room and a description of the events was given to both the patient and his family member. The plan now will be to monitor the patient on the telemetry monitor for approximately 1 hour and if sinus rhythm is maintained then the patient will be discharged.
Follow-Up:
The plan will be to follow-up with Dr. Z as an outpatient.
